The Fabric of Our Community
- I am the parent or legal guardian of the child/young person listed.
- As the parent/guardian, my contact information has been completed in the submission form.
- I attest that the artwork submitted was created by my child.
- I agree that the art may be used at any time to advertise The Fabric of Our Community Call for Art and Art Exhibit by the Willoughby Arts Collaborative and project partner agencies - including but not limited to advertising through news media, radio, television, social media, flyers, posters, etc.
- As the parent/guardian, I am in agreement that the collaborative team may list the art title, artist name, and affiliated school for purposes of this project.
- I understand and will abide by the rules and guidelines here within.
